What do I need to bring to my first appointment?

Please print and fill out our new patient forms by clicking on this link. Once you've filled them out, please send them to us prior to your appointment by either faxing or mailing them. Our fax number is (480) 452-1123 and our mailing address is, 3501 N. Scottsdale Road, Suite 140, Scottsdale, AZ 85251. This will help minimize your wait time, and get you in to see the doctor faster.

We also ask that you bring your driver’s license or picture ID to your appointment, along with your insurance card, and any co-pay or deductible that applies. Please also bring copies of any prior test results.

What kinds of conditions do you treat?

Dr. Zacher specializes in minimally invasive treatments that offer less pain, less scarring and quicker recovery time compared to traditional surgery. Dr. Zacher also performs open surgical operations for those conditions that require traditional surgery. He has special expertise in minimally invasive treatment for many conditions, including hernias, acid reflux disease, and gallbladder disease. He also frequently deals with cases related to Worker’s Compensation.
